Summit NJ. said I had a balance from a recent appt to City MD

Message verbatim “Scam alert: We only call from our official number during business hours. If you get a su****ious call, don't share info--contact us directly. Thank you! Reply STOP to opt out” I however never interacted with this number prior and I don’t even wanna message reply because then they will know the number is active.

Said they were from my Dr. group, MSPB and wanted me to fill out a form asking for my SS number and birth date

This was a text supposedly from a doctor's office about a bill. We do not know anyone in New York City and certainly have not gone to a doctor there. My husband did have a heart attack in September while we were at his brother's funeral in Illinois. But NOT New York.
