Part of my physical in '09 was with Mid-Atlantic Eyecare, w/ Steven K. Snyder, M.D.  I contacted them by the phone number on the front of the card - 757.***.5130 - on how to get a copy of my records for this years' physical sent to my new doctors.  After ****ing up I realized I needed an address, and it's not on the front of the business card, so I checked the back - Mid-Atlantic has 3 offices in Hampton Roads, VA:  Norfolk, Va Beach and Chesapeake.  The addresses are listed there, too.  At the top by the word 'Offices:' is written the number 646-0817.  The number printed on the front is different.I have neither called this number, nor had a call from this number; I only researched it via Google, wondering what the number meant on the back of the card, since there's a perfectly good number on the front (it could be a clue which office I attended) and wound up here.  I'm also not pointing fingers - my care at this facility was great.  But what the guy is doing is a bit unsettling.  FYI
